When applied topically minoxidil stimulates hair growth in persons with male pattern baldness.The exact mechanism of action in enhancing hair growth is not known. However, it may act through various mechanisms as given below:
• Enhancing microcirculation around hair follicles through its vasodilatory effect.
• Minoxidil stimulates mitosis in epithelial cells and prolongs the survival of epithelial cells in tissue culture. It has therefore been suggested that the drug slows the ageing of hair matrix cells.
• Alteration of the effect of androgens on the genetically predetermined hair follicles. Minoxidil may affect the androgen metabolism in the scalp or inhibit the capacity of androgens to affect the hair follicles.
• There is no evidence of follicular neogenesis, rather new hair comes from existing quiescent terminal hair follicles and from vellus follicles that may have revived.
• Only those vellus follicles that have not advanced to total senescence are capable of growing hair.
